Your Claim Form
Select the relevant claim form from the list below, and print
out from the pdf. Alternatively, ask the QBE claims department
or your insurance adviser to send you the required claim form.
Submitting
Your Claim Form
When you lodge your claim form, we need a signed declaration.
For that reason, you should print out your selected claim
form from the pdf's above, then submit the completed form
by mail or fax.
Alternatively, you can scan in your completed and signed
claim form and email it to our claims department - see contacts
for details.
Monitoring
Your Claim's Progress
We will advise you on the progress of your claim through your
insurance broker or agent - or directly. You can also contact
our claims department for an update, if you like.
Future Strategy
- Electronic Submission
In the future, our plan includes on-line claims forms that
you can submit electronically, and claims monitoring that
can be done over the internet.
