You can report your claim any of the following ways:
Complete the Healthcare report form (see below for instructions and link), and send it to NewLossQBE@us.qbe.com
Print out and complete the Healthcare form below, and fax it to 844-879-2459, along with any documents you have that relate to your claim.

Healthcare form instructions:
- Click on the link to the PDF form above
- Complete the form with as much information as you have available. Note: electronic signature is optional
- Save the completed form to your computer.
- Open your email application (e.g. Outlook, Gmail)
- Attach the form to a new email. Also include any documents you have on hand that are related to the claim.
- Send the email to NewLossQBE@us.qbe.com
- A claims professional will be in touch with you shortly to discuss your claim
