Report a new Cyber claim or Cyber event

You can report your claim any of the following ways:

  1. Contact your broker, who will report the claim to QBE on your behalf
  2. Send an email with the loss information outlined below to Professional.Liability.Claims@us.qbe.com
  3. Call QBE at 844-723-2524 (844-QBE-CLAIMS), and select option #2 to report a new claim and then select option #4 for liability claims.
  4. Fax the loss information outlined below to 844-879-2459

Loss information required to report a new claim via email or fax:

  • Policy number
  • Insured name and address
  • Contact information (primary insured contact for the claim)
  • Date of loss/incident
  • Description of loss
  • Claimant/event information
  • Facts surrounding the claim/event
  • Any documents related to the claim (e.g. notice of lawsuit, customer communications)
  • Date that the insured became aware of the claims/event
